YOU MUST CALL 3 BUSINESS DAYS BEFORE APPOINTMENT – MONDAY-FRIDAY 8AM-5PM

  1. Call First Transit for a list of transportation companies that serve your area.

  3. If a company agrees to provide transport, next you call First Transit at 1-877-725-0569. You will request an authorization number for transportation to each appointment. You will need to provide the following information:

  5. Say “I am calling to get authorization for transportation to a medical appointment.”

  6. Tell them your name:

  7. What the appointment is for: (i.e. labs, follow up, procedure, etc)

  14. They will give you an Authorization/Reference # (write it down):

  15. After you have the Authorization or Reference #, call transport the company back to provide the number and confirm the rides for your appointments.

This is only good for ONE ride, roundtrip. Follow the same steps for each ride. You must call three days in advance of the appointment.
